~PROTECTIVE EYEWEAR IS REQUIRED TO TAN WITH EnlighTAN Day Spa~ EnlighTAN
Day Spa provides free, disinfected eye-wear for each tanning session. Eyewear is also available for purchase.
EnlighTAN Day Spa provides free literature on the safety aspects of the tanning industry. Please ask a helpful staff
member for this information.
If you are under 18, you MUST have a signed parent/legal guardian
consent form to tan at EnlighTAN Day Spa This form is to be signed by the parent/legal guardian at the salon
and approved by management before you can tan. If the parent/legal guardian is not able to come in personally, a Notarized
form by a Colorado Notary will be accepted for approval.

If you must cancel an appointment, please do so within 24 hours of your scheduled
appointment time or you will be charged for the time allotted.
Loud and/or profane language will not be tolerated
in EnlighTAN Day Spa.
Disrespect to another client, or staff member will not be tolerated.
Disrespect
from a staff member toward our clients is prohibited and is grounds for immediate dismissal.
Payment Methods
EnlighTAN Day Spa accepts the following forms of payment:
VISA, MASTERCARD, AMERICAN EXPRESS,
DISCOVER, CASH AND PERSONAL CHECKS.
EFT Declinations and personal checks that are returned will be charged
a $30.00 NSF fee and your membership will be cancelled unless full payment is made within 7 days of notice to one of
our managers. EnlighTAN Day Spa reserves the right to refuse further services to anyone who has defaulted payment of
any type.

file information and transactions at EnlighTAN Day Spa This includes information pertaining to gift recipients. Financial
information including credit card numbers, expiration dates, billing address is used solely to bill the customer for their
order.
